Creating a plan of care is the first step to building a strong partnership between Homewatch CareGivers of West Hartford, and any new client.
We begin our relationship by getting to know each client, their family, and extended support community, if there is one. In our first meeting, we’ll seek to understand the needs of the client and explore social history and primary interests. The plan of in home senior care is developed collaboratively with the client and family and is designed to:
- Act as a guide and basis for the client-caregiver partnership
- Help the Homewatch Caregivers team to schedule and match a compatible caregiver (s) with the client
- Become an orientation tool for the caregiver (s) to outline expectations and approach to care, prior to beginning services
- Document our agreement around scope of the services
