Nassau County Plan of Care
A plan of care is a reflection of a dialogue between our clients, their
families, our caregivers, and our staff in Nassau County. One of our clients,
who was a retired social worker, was involved with her daughter in building
a care plan that included exercise, doing puzzles, and making meals together
to ensure a busy, rich and eventful day. This plan factored in paying
attention to balancing a quality of life and good health. For those using
New York's Consumer Directed Personal Assistance Program (CDPAP), your
personally selected friend or family member who is assisting you, will
be able to structure a plan of care just for you.
