El Paso Plan of Care

There are different elements that go into creating an in-home senior care plan for each client in El Paso. For our clients who are using Medicaid, we look at their circle of family and friends and neighbors and ask who they feel would be a prime prospect to be a paid caregiver for them. Not everyone is using Medicaid and our staff of caregivers is available to meet the needs of our clients. We also look at partnering with local long-term care resources in the community such as the Senior Companion program and Veteran’s Care Coordination so that these people can reduce the expense of home care where possible. This plan of care factors in all of their needs so it might also include putting them in a touch with a local non-profit food pantry or non-profit home renovation service. We are assessing all of their needs, not just the list of personal care needs.
