The plan of care is one of the most important documents in providing senior care in Chapel Hill. The care plan also includes details about client history, family dynamics, and any special instructions regarding their care. Periodically, our nurse makes quality assurance visits to the client’s residence and updates the care plan as client’s needs change. One copy of the most updated care plan is always kept at the client’s residence for the family to review, others are given to the care team involved in the care for the client.
