Chapel Hill Plan of Care
The plan of care is one of the most important documents in providing senior
care in Chapel Hill. It is designed in close collaboration between our
nurse, family members, and any other healthcare professionals involved
in the care of the client. The care plan is devised to reflect the needs
and wants of the client and the family, covering anything from simple
companionship to complex personal care. It is made to reflect the length
and frequency of services, types of tasks (such as bathing, dressing,
grooming, meal planning and preparation, transportation, shopping, exercise
assistance, medication reminders, etc.), and the duties that the caregiver(s)
is required to perform to keep clients happy, safe, and comfortable. The
care plan also includes details about client history, family dynamics,
and any special instructions regarding their care. Periodically, our nurse
makes quality assurance visits to the client’s residence and updates
the care plan as client’s needs change. One copy of the most updated
care plan is always kept at the client’s residence for the family
to review, others are given to the care team involved in the care for
