We work with a lot of home health and hospice companies in our valley. Most of them do a great job. One of the companies in particular we have worked with regularly is Horizon Home Health and Hospice. They provide similar services that other home health and hospice companies provide such as physical and occupational therapy, skilling nursing, bath aides, social workers, chaplains, etc. They also have a company called Finding Home which provides house calls for home centered primary or palliative care in the home. They are a certified palliative care provider which Dr Derrel Walker oversees care as the medical care director.
